You may have the opportunity to purchase benefits on a spouse’s plan. If your primary plan has an exclusion for weight loss surgery, your secondary plan may cover your surgery.

Diet and Medical History Forms
You will need to complete a set of new patient, diet and medical history forms for your surgeon to review. You can download those here.
Psychological Evaluation
You will be required to have a routine evaluation performed by a licensed psychologist or psychiatrist. We can provide you with a referral. After you’ve selected a doctor, we will provide guidelines for the completion of the evaluation. The evaluation must be completed prior to surgery.
Primary Physician Letter
You may be required to obtain a letter of medical clearance from your primary care physician, as well as a TSH level (thyroid blood test). This letter should include your health history, any co-morbidities for which you are being treated, any previous attempts at weight loss, and your doctor’s opinion as to your suitability for bariatric surgery.
Other Information
Your surgeon or insurance provider may request other information or other documents to review your case. We will review all of these extraneous requirements with you and help you obtain them. However, you will ultimately be responsible for providing this information.
